Joint replacement is not a first step. It is what follows when cartilage has worn through, the joint surfaces are grinding on each other, and the measures that came before — activity modification, physiotherapy, medication, injections — no longer keep the pain in check.
The decision rests on function more than on an X-ray. How far can you walk before you stop? Can you manage stairs, dressing, prayer? Is the pain waking you? An X-ray showing severe wear in someone still managing their day is not on its own a reason to operate.
Where arthritis is confined to one compartment of the knee, a partial replacement can be an option: it preserves healthy bone and the cruciate ligaments, and the knee often feels more natural afterwards. That depends on the pattern of disease, which is why it is assessed case by case.
Modern knee and hip replacements last 15 to 25 years in most patients with appropriate activity levels. Recovery varies from patient to patient and is supported by physiotherapy from the first days after surgery.
