Background
Shoulder surgery in Peshawar
The shoulder trades stability for range of movement. It is the most mobile joint in the body, held in place by the rotator cuff, the labrum and the surrounding capsule rather than by bone. That design is what allows an arm to reach overhead, and also what allows the joint to dislocate.
Two patterns bring most patients in. The first is a rotator cuff tear: night pain, pain lying on the affected side, weakness lifting the arm. The second is instability: a shoulder that has come out of joint once and now feels unreliable, or comes out repeatedly.
Both are assessed clinically and with imaging. In recurrent dislocation a CT scan matters, because bone loss at the front of the socket changes the operation: a soft tissue repair alone is likely to fail if enough bone has been worn away.
Small rotator cuff tears often settle with physiotherapy. Larger or complete tears usually need repair, and leaving one untreated allows the tendon to retract, which makes later surgery harder.
